New EDW student athletes in grades 8–12, including those new to EDW or participating in athletics for the first time, must print, complete, and submit all forms in the New EDW Student Athlete Forms (Grades 8–12) packet.

Returning EDW student athletes in grades 9–12, who have previously participated in athletics, are required each year to print, complete, and submit the Returning EDW Student Athlete Forms (Grades 9–12), which include the LHSAA Medical History Form and the Athletic Contract.

Links to all required forms are provided below, and all completed forms must
be submitted using the Google Form link before participation in any sport.
